Forms: School, Camp, or Daycare

Fall Hill Pediatrics has made our school, camp, and daycare forms an entirely electronic process.  Please send all requested forms back to us with parent portions completed to our office email (Failure to complete the required portions may cause a delay in the completion of your forms.)

​Please be advised, if you submit multiple forms for one child or for multiple children, you may receive them at separate times or on separate days. All of your forms are in the queue to be completed.​

• Standard turnaround time: 5-7 business days

• We do try our best to complete your forms during your visit.  However, sometimes this may not be possible due to high patient volume, staffing shortages, or busy seasons.  Summer (May-August) is our busiest time of year for form requests.  We highly recommend not waiting until the latter part of the summer to request completion of your forms.   

• ​​​Business days DO NOT include Saturdays, Sundays, office closures due to unforeseen circumstances (i.e., inclement weather) or major holidays. Major holidays include: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the following Friday, and Christmas Day.

 

​Below you will find the most commonly used forms.  We do recommend reaching out to your child's school, camp or daycare to ensure we are filling out the correct forms.  Some schools, camps and daycares have specific forms they require and will not accept the forms below.
